About Xeljanz 

Xeljanz is a disease-modifying anti-rheumatic drug (DMARD) that belongs to a group of drugs known as Janus kinase (JAK) inhibitors. It functions by interrupting specific signaling pathways and suppressing the immune system.

Some conditions that Xeljanz is known to treat include rheumatoid arthritis, psoriatic arthritis, and ulcerative colitis. Xeljanz is available only by prescription. It is available in a generic formulation as well as under different brand names, including tofacitinib.

Xeljanz may be prescribed as a tablet, oral solution, or extended-release tablet. It’s always important to follow the specific instructions on your prescription, as they can vary based on the formulation and dosage prescribed.

If you're prescribed Xeljanz, be sure to complete the full course of the JAK inhibitor unless your doctor specifically tells you to stop. If you don’t complete your prescription, the JAK inhibitor may not fully treat your rheumatoid arthritis, psoriatic arthritis, or ulcerative colitis.

  • Ulcerative colitis

    Xeljanz is FDA-approved to treat adults with severely active ulcerative colitis.

Xeljanz side effects

The side effects associated with taking Xeljanz are typically mild. Most Xeljanz side effects are related to upper respiratory tract infections.

  • Xeljanz has some common side effects. They may include:

    • Upper respiratory tract infections (common cold, sinus infections)

    • Headache

    • Nausea


    Other side effects, which are less common but might be more severe, could include:

    • Serious infections

    • Cancer and immune system problems

    • Increased risk of major adverse cardiovascular events


    You should call your doctor if you notice any of these side effects, or if you develop any other new or concerning symptoms. Xeljanz can increase your risk of developing infections, including urinary tract infection (UTI), upper respiratory tract infection, and skin infection.

    Xeljanz may be used in young children over two years of age to treat polyarticular course juvenile idiopathic arthritis.

How to take Xeljanz

Xeljanz can come in different forms and doses, so be sure to follow the specific instructions on your prescription. The starting dose is typically Xeljanz 5 mg twice daily for arthritis and Xeljanz 10 mg twice daily for ulcerative colitis. Your doctor may adjust your dosage if you have severe renal impairment or moderate hepatic impairment.

Xeljanz is typically taken twice a day with or without food, while Xeljanz XR tablets are taken once daily with or without food. Follow your healthcare provider's instructions throughout Xeljanz treatment.

What to avoid while taking Xeljanz

Xeljanz has 531 drug interactions. Don’t change what you are taking without checking with your doctor or pharmacist. That includes other medications or supplements, as well as over-the-counter drugs.

  • Other medications used to treat rheumatoid arthritis, psoriatic arthritis, or ulcerative colitis

  • Other immunosuppressive drugs

  • Medicines that affect the way liver enzymes function

Epstein Barr Virus-associated post-transplant lymphoproliferative disorder has been observed in renal transplant patients treated with Xeljanz and concomitant immunosuppressive medications.
